Medical Home Campaign Development

The Foundation's Medical Home Campaign actively promotes the importance of establishing consistent preventative, chronic and acute health care for children. The program works to educate the public on the importance of identifying a single, consistent source of medical care for children, whether it is a clinic, primary care physician or urgent care center.

Background

  • A Medical Home is the one place you take your child for all of their health needs
  • Oftentimes, children are not consistently taken to the same source for medical care, and as a result do not receive consistent care from medical providers
  • Repeatedly visiting the same providers helps ensure that staff members are familiar with the child, their health and their medical history
  • Establishing a relationship with a Medical Home also helps eliminate unnecessary visits to hospital emergency departments

Goals

  • Increase awareness of the importance of establishing a Medical Home
  • Decrease the number of unnecessary emergency room visits by providing highly informative educational materials detailing common child health problems

Audiences

  • Expectant parents, parents, and grandparents of young children
  • Social service representatives, community groups, and medical professionals interacting with, and providing information to, parents and their children
  • Education professionals who interact with parents and their children

Campaign Activities

  • The foundation of the Medical Home program is the development and distribution of bilingual (English and Spanish) educational materials. An introductory brochure outlines the importance of establishing a Medical Home and a series of informative bookmarks address common child health ailments, including instructions on treatment and emergency room use.
  • Materials are distributed free of charge via the Foundation Web site, mass-mailings, through professional exhibiting and conference opportunities and in response to catalog orders
