Health Check (medicaid for Children)/ NC Health Choice

Your child may qualify for NC's Health Check (Medicaid for children) or NC Health Choice (CHIP) - free or low-cost health insurance for children and teens under 19 years old*. Even children in a family of four with working parents earning $51,276 a year or slightly more may qualify. These programs also cover children living with grandparents, other family or friends. Your children need a valid social security number to qualify for these programs. Families with children who are U.S. citizens or lawfully residing in the U.S. may be eligible.

*NC's children in foster care on their 18th birthday may have Medicaid coverage until the age of 26.

To apply online via secure web site, go to There, you may apply directly using ePass (online) or download an application. You may complete online. To apply by mail, download, print and complete the application. Mail the completed form and copies of requested documents to your local (county) Department of Social Services (DSS.) Your local (county) DSS may contact you with questions.

Stay Enrolled!

Your ID card will be mailed to your address on the application within 45 days. Once your child is covered by this insurance, it will last for 12 months. Any changes to your household income will not change the insurance. At the end of 12 months, DSS will review your household situation to see if your child still qualifies. The caseworker will let you know (sometimes by mail) if any additional information is needed. Please read all mail you receive from DSS or Division of Medical Assistance (DMA.)
